Appointments & Billing


You may schedule an appointment by calling (408) 356-6650 or email an appointment request by selecting one of the following options:

Arrive on time: As you know medical/dental offices sometimes run late. This is primarily for three reasons: emergencies, difficult procedures that require more time than anticipated, and patients who arrive late. We make every effort to see you at your appointed time. If there is an emergency or unanticipated difficult procedure, we will notify you upon arrival. Your appointment time is reserved for you, please arrive on time as late arrivals backup the schedule for subsequent patients.

Broken Appointments: If you need to change an appointment please give us as much notice as possible. If you must change your appointment, we require at least 24 hours notice to avoid a $50.00 cancellation fee. (emergencies are an exception).

Emergencies: If you are having a dental emergency like a toothache, injury, or swelling, we can schedule you for a limited exam and an x-ray. We will diagnose the problem and get you out of pain. We will also give you a treatment plan that shows the work that needs to be done and the associated cost. Because emergencies are usually worked in between our regularly scheduled appointments we can't guarantee that we can complete the whole treatment on that visit. We will at a minimum prescribe necessary medication to help alleviate the pain. We will do our best to complete emergency treatment at that time if possible.


Insurance: We accept most insurance plans. We understand the value of your insurance benefits and will assist you in obtaining your maximum benefit. We will gladly process your insurance claim for you and will ESTIMATE your deductible and portion that is not covered by insurance. That amount will be due at the time of treatment. Our estimates are subject to final approval by your insurance company and could therefore change the amount due our office. Our estimates of your coverage are just that, an estimate. We are usually very accurate. Occasionally insurance companies deny claims that we did not predict, in these instances the remaining balance is due from the patient. We will be happy to resubmit claims on your behalf if necessary.

Payment Options: Our office has eliminated billing, therefore we do not send out monthly statements. We ask that you pay your ESTIMATED co-payment at time of service. As a courtesy to our patients we accept insurance assignment, but this does not absolve the patient of responsibility for charges in full for the treatment rendered. The estimate provided by our office is considered to be a guideline until the final insurance payment is received and the patient's account is reconciled. This office can make no guarantee of the insurance payment  estimated. We make every attempt to be as accurate as possible.

2. Visa, MC, Discover, AMEX:  we accept credit cards as payment for dentistry as your limit allows

3. CareCredit: We offer a separate line of credit to cover your entire family's dental needs. Approval takes only a few minutes. CareCredit does not affect your current spending limits and balances on your other credit cards. There is no annual fee and INTEREST FREE options are available. We would be happy to work with you to plan the most appropriate arrangements for your budget.
